How I do this is, let the Papal States consume Urbino, and others, add Naples, and give everything else to the Holy Roman Empire, Venice is also independent, but I made that out of Croatia,
Don’t worry San Marino still have the exact same borders

As for Malta, I would make it a releasable from the Crown of Aragon, since it was apart of it.
But I would change its name to the Kingdom of the Knights and change its flag to this first.
