You've got a few options
One technique you could try is to use a live tuning pitch correction effect with MIDI support

So it tunes it according to a midi?

I can't find one about setting up MIDI Out, but the idea is that you'll choose a port number, set it to broadcast notes out of that port, then set up GSnap to recieve notes from that same port in the wrapper settings
