Some people won’t return.
They will leave you and never come back.
But that’s not your concern,
Some trains are destined to switch tracks.

You will find the train to board,
The one that won’t change destinations.
You will be able to walk aboard,
Not worrying about losing foundations.

The right person’s map will never change,
Refusing to suddenly abort its directions.
Your name will stay on that page,
Even with stress like the worst infections.

The right person won’t have to return,
They promise to stay, worries overturned.

It is a very deep poem clearly produced by emotions shaped from personal experience. The use of "you" gives the subject a connective bridge with the receiver. Establishes that this is an experience us too will have in time and to not get discouraged.
My only thought is that the last two lines don't rhyme like the rest of the poem. Maybe you could modify them to rhyme better and perhaps make it four lines too like the rest of the poem. Overall it was a very good poem, I enjoyed reading it and it really touched me.
