If you want to make your poems more unique,
- Use abstract imagery (like Carol Ann Duffy's Valentine)
- Use a unique writing style (just make your own)
- play with diction (since you said every poem you write seems similiar)
- Add different themes instead of just love (lovebreak, slowlove, love at first sight etc)
- Play with rhythmn (rhyme schemes: Terza Rima, Sonnets, Ballads) and enjambments
- Review your poems with others and get feedback. (Thats how you grow and learn styles)

For me what made me able to convey a more profound is the expansion of Imagery and use of different words
I mainly write poems abt my gf so , write about your unique experience with your partner ig? What changes they have brought to your life etc etc you can't write love poems of you ain't in love
