What version of Minecraft are you using?
going from 1.21.3 to 1.21.4

In 1.21.4, there are a few extra steps you have to go through to use the item_model or custom_model_data component.
If you already have a resource pack working in 1.21.3, and have some experience with resource packs, this video will show you what you need to do to get your custom model working with the item_model component.

Do you want to use item_model or custom_model_data?
If you're looking to use item_model, the carrot_on_a_stick.json file is uneeded.
In fact, you technically don't need anything in the minecraft namespace of the resource pack at all.
I assume its just the item_model string id

so I would do like give carrot_on_a_stick[item_model=minecraft:<custom_item_model>]
Yes, but you don't need the custom model to be in the minecraft namespace. What I did in my pack, was put my model file in a custom namespace, and then the component looks like item_model=custom_pack:my_model_file.
The video should cover this in enough detail to aid in comprehension.
