Introduce a simple old school RPG-vibe adventurer's log 2D portrait/avatar for players' character. A few (20 or so) could be premade ones from the Developer team (the picture of a generic warrior, wizard, archer, ect.), while if a player wants to get a custom portrait/avatar or upload one him or herself (the picture/image must follow the medieval Old School vibe/theme of the game), they must pay in-game gold per week/month to maintain it. Each time a player wants to change their custom avatar/profile, they must pay in-game gold too (at the same price as getting a new custom character portrait/avatar).

This is a good and fast method of gold sink, is easy to introduce in-game, and does not detract from the game's core mechanic/gameplay

This is similar to how simplemmo has their avatars, where you need to follow some guidelines and pay diamonds (premium currency) for custom avatars or profile banners. They don't have to pay to change them each time after they're in your collection you could change to whichever one anytime. They can also be sold and traded on market.

This would add customization for players to show off their profile pages etc. but it's a big project that would change the theme of the game for better or worst. I'm not sure if this should be added just for a gold sink though.

Probably the gold sink idea is a bad one, but I still recommend a simple-to-code "write your bio" and portrait/avatar feature to facilitate player-to-player roleplay/RP. Both RuneScape and OSRS have this feature to keep players hooked to the game when they are waiting for their afk (in our case, idle) grinds. It would give the opportunity for future players to write about their in-game character, e.g. Zaros is a demon slayer from Lumbridge, Kate is a female wizard, ect.

indifference. leaning towards no. its an idle game anyway. most of the time offline.

You're right, it's better to focus on other things, like maybe, quests, more interactive content, lore, ect.
