I've gotten to Maxx on my run using the new safety guidlines mode and each time I die, instead of showing the death screen, the game shows a blurred screen with only some lights and the boss's health bar (screenshot added). The music still plays, no buttons work. I had to quit using task manager. Trying to restart the run puts me in a loop of the loading screen overlayed over me still being able to control the character.

Didn't happen with previous deaths on the standard difficulty (One to Brad and one in the Industrial department)
The bug started happening when I got to Addictions trying to check out how safety guildlines works.

I come to give some clarifications because I have the same worries as Lun; it comes from the easy mode I think, since I activated it to unlock a weapon, I died against the second boss Güdron and the burnout screen was not displayed and the boss was attacking in loop. I quit and restarted the game; I appear before the fight and beat it; now here in the video the problem is further into the game and even against simple enemies the death screen does not appear.
