Beacon of Hope didn't drop!

Hii!! I have questions about Beacon, is it 1 per account ? 1 per character?

I rushed a friend and killed baal with him standing next to me & the beacon didn't drop.

This wasn't my experience when I was rushed, curious what I have done wrong or what I can do to get another one ?

I've given him mine and was curious if I'd have to re-get to baal on a new character to get it back?

I failed to find this info on this page https://www.d2rmodding.com/remodded and hoped that someone might have more detailed info so I don't walk 5 acts to find out.

Try in normal mode. Tried in Hell many times and no luck. First time in normal, and it dropped.
