Up to Ext, the difference is

• having the baseline roster like Akane, Maki, Ako, Kotama, a tank, a healer etc
• having baseline invests (EX3/5 where applicable, T4 gear or higher, level 70-ish etc)
• having access to good DPS borrows
• knowing how to deal with boss mechanics
• knowing how to rotate skills

Failing Ext means you failed one or more of these
