My leader has smackback and was hit by both Truffle and then Puffball. The result was a single return attack on Puffball, but nothing on Truffle.
My only guess is this is possibly because both the leader and Puffball had 0 
and the softlock prevention kicked in?
But even if that is the case, I assume it would still process the smackback for the next target 😕

Truffle split here due to the first overburn hit, meaning there was probably jank to do with the hero unable to tell which truffle to hit and as such hitting neither

Ah, I should have probably taken a screenshot of the whole battle, but I'm like 90% certain that the Truffle that split was separate from the one that attacked.
Truffle was in the top row with my leader, but the Puffball was in the bottom row.

Interesting, I would have assumed that the Smackback would have attacked Truffle before Puffball triggered

According to other posts I've seen. I haven't personally encountered it (until maybe now?)

it's for when smackback triggers against smackback constantly, not just for 0 attack smackback

That makes sense. Then it is unlikely the cause of this bug.
