Itemsync is to sync items when playing with someone during a randomiser
Usually if you don’t know what mods do, the readme of the mod will explain what you need to know

you need the randomiser mod for it to work
but you can minimise the effect of the randomiser mod to have an experience similar to what you want

HKMP only syncs players and their movement by default (learn more), but you can install additional mods in Lumafly or Scarab Mod Installer to make things mostly fully synchronized.
