i did some research
basically a tarnished is someone who's lost the grace of gold
the grace of gold ≠ the guidance of grace
since tarnish means for something to lose its luster and whatnot so like the grace of gold was dimmed in the tarnished's eyes
that's how npcs immediately tell that you're tarnished because you eyes are different/dimmed or something but that's only my best guess someone more educated could say I'm wrong

red eyes from finishing varré's quest and becoming a bloody finger, yellow slitted eyes from eating dragon hearts, madness or something eyes from embracing the three fingers
they overwrite eachother though and you cant get them back that's what i found

since when i ate enough dragon hearts, i got the slitted eyes, then got the frenzied flame and that overwrote my dragon eyes

no option to get them back in the closet 😢
