There's no box to obtain their items, no icon, nothing. They're just gone from existence with everything they had on them.
Anyone else run into this issue?

Just happened to me as well and I've only been in relatively safe realms or my Abeyance. Just before I noticed she was gone, I also noticed that her health bar was dropping every time I moved between realms.

So I had one dissapear mid-battle - in a swamp-gloom realm. I hired another and outfitted him, and went to another realm. this one died and converted to a chest right in front of me. So now I need to go back and see if my first NPC's chest is there someplace. I did not realize they could die so easily, especially with good gear. Granted we were being attacked, but I guess i expected them to just crouch down and wait to be revived, not die outright?

I've never seen mine die and I've soloed Jana. Get reduced to needing to be revived? Sure. But not death.
It sounds to me like a bug of some kind and it's interesting that you also had it in a swamp.

since both times it was during intense battle, I am left wondering if I accidentally hit my NPC and that did it? I have no idea how the mechanics work

I've smashed mine repeatedly while harvesting trees and lumber and never seen her health even move. Don't think that's it.
