Making a report adds it to the player's permanent record; however, Riot does not disclose whether your report resulted in a ban for the player. All I know is that a report makes a difference whether it's made using in-game / through player support. If you want to submit evidence do it through Player Support, as they look into it thoroughly and is more effective afaik.
