The log is incomplete, and not providing the error only in a screenshot form makes it a little more difficult to find in this log.
Plex told Kometa that these three shows are in the TV library:

But the internal IDs that Plex gave Kometa apparently don't exist in Plex.

The other two fail the same way.
This is either an issue in Plex, or bad information from Plex at some point which Kometa cached.
All three of these shows are newly added to the Kometa cache on this run, so that argues in favor of some breakage inside Plex:

Okay - just done a db repair - nothing has changed with those titles, I was having issues with this for a while so I removed the cache guessing why they're showing as new

Found the problem
In the Config on Kometa I had split duplicates to true, and it was trying to split those failed ones
Can you elaborate on this a bit more?

I've never personally considered the impact on mapping IDs when duplicates are split
I had it enabled on my config originally and never removed it, but updated Kometa a few weeks ago and have had the issue since, but since tried it in isolated environments and had the same issue with that version, but as it was just me never really raised it - I can't speak to why it breaks, just that when I run kometa it gets to the splitting duplicates and then fails not long after. When I disable the split it never gets to that stage and skips over it and nothing fails
I have ran a db repair, but it had the same issue after that
And we are talking about the 404 error yeah?
It would be worth us reviewing the Kometa code to see how it handles IDs when duplicates are split
I don't know that the cached IDs are updated when the operation runs to split dupes
If a new version of a movie is added, I think Plex automatically merges them right?
That may have an impact on the IDs that Kometa uses
Yeah if I have two versions of the same movie it puts them together
Splitting duplicate TV is probably odd to begin with since the actual duplicate is an episode file way down inside the show; not sure what actually get deduped and how Plex handles it.
