A customizable survival mode allowing players to tweak their experience to their preferences would add a large amount of replay ability to the game.

Inventory settings: Settings for how players store items on themselves or in containers. Ranging from casual like TWDS&S with easy to manage slots to place items directly into with no regard for the weight of each item, to having more hardcore inventory management where you have to manage the backpack space and weight, put individual pouches on your character for carrying ammo, and attach every weapon to the player in some way regardless of if it’s a sling, holster, sheath, or some other way to attach the weapon.

Healing: healing in much the same way as the inventory would need different levels of realism, with things ranging from being able to heal from any wound without treatment but being able to speed up the process with basic healing items and potentially even food items, to having to manually stitch up, clean, and bandage wounds, potentially even having to set broken bones back in place to heal wounds, or being able to lose limbs entirely to permanently lose functionality until dying and respawning on the furthest end of realism.

Basic needs: The basic needs of the player like food, water, shelter, and hygiene are another thing to take into account, some players will want a very casual experience where they either don’t need to eat anything and all food items act as healing items, or it helps with stamina. Some players will want basic needs that are required but aren’t too strict like a basic food and water requirement, while some players will want to have to fully immerse themselves into the world, cleaning off grime to avoid getting sick, wearing correct clothing for the environment, and trying to meet basic nutritional needs.

AI: to keep the game feeling fresh, AI will be a necessity, and allowing players to pick what they face will allow players to change their experience to fit what they want. There’s the basics like animals and zombies, but some players might only want animals or zombies. Furthermore, some players might want bandits in or friendly survivors to mix up encounters, and some players might just want to have some dumb fun and fight something like furries using the mode to have the dumb fun of the sandbox while still having some basic progression.

Item spawning: the items players use and collect will need a way to be found, and so loot pools that players can edit themselves would allow players to tailor the experience to what they want.

Premade settings: premade settings for players to choose from, with settings made by the community available, would allow players to pick an experience that suits what they want as closely as possible and jump into the game with little to no settings changes, with things like a maps standard loot pool, or basic survival difficulties.
