Mechanics such as pull out damage where a blade will do initial damage but won’t kill you (as long as no major area has been hit). With need to use a healing item to actually stuff your wound or if you pulled it out you would bleed to death slowly . This could be an optional setting within sandbox or the tactical game modes lobbies. For objects such as bullets you could possibly have some highlighted area where you would have to grab and pull a bullet out (this being less realistic as some bullets could shatter and spread or be too deep maybe the deeper it is the harder you have to pull). Npc’s will have to do this as well but would have to coded to actively seek out someone to guard them, this being fairly hard to code I assume. This could be a huge stretch but I think doing something like this would be pretty interesting and innovating with the only similarity I can think of is green hells mechanics.
