When a player takes damage or is pushed enough to make an npc fall over, they get blurry vision while they fall to the ground, then when they hit the ground they can see again, and if they reach a certain height above the ground by either being helped up or pushing themselves up, they enter a getting up animation. And depending on your health and the severity of your injuries, it will be harder to get up. Another thing to make would be to let players grab each other. If you are grabbed on a body part, you both should feel haptic feedback that gets stronger as you pull away from them until you break free, or they break your bones.

at least breaking the neck is confirmed in multiplayer, at least thats what the video said
so I´d imagine the rest might work too
But the whole ragdoll system
