If u don't use breath hold and go to poison effective range or in water your health will immediately start decreasing.
This makes poison grenade more used by players also without manually holding breath u are unable to camp in underwater for too long.

Also there is already a icon indicator for breath hold they just need to make it used by manually.
That means there is no extra button needed.

Also without hold breath implementation weapon scope sway can't be added properly because after opening the scope aim will start moving and it makes aiming harder if u want to stop the movement u need hold breath if there is no hold breath function u need to close scope and open scope again and again for better aim.
