Your mods folder is located at .minecraft/mods

Use VBOS: ON
Smart Animations: ON
Render Regions: ON
Smooth FPS: OFF
Smooth World: OFF
Fast Render: OFF (for mods that need it off)
Fast Math: OFF (since this breaks things more than it helps)
/patcher > Search HUDCaching > enable it

your render distance isn't set to 64 right? that happened to me once lol
