Nope. No one gets it at all at the beginning, in fact it's considered a very last-case scenario.

Depending on the local practices, they may either first prescribe a non-stimulant (no effect on dopamine/no addiction potential), or they may prescribe methylphenidate (its the weakest medically proven stimulant)

Also they have to follow a dosage increase. You start from the minimum effective dose, and they slowly increase it over time. They also prefer to give stimulants in a sustained/extended release form, to reduce abuse potential

Before I mention anything, I took all the below after conversations with my psychiatrist

I occasionally take melatonin, I have a 3mg tablet for when the stimulant meds keep me awake and I don't wanna sleep too late (very terrible days), I have a spray that's 0.5mg per spray, I take that depending on the day/feel, and take magnesium glycinate daily

The magnesium one helped me with sleep related restlessness/overthinking (daily usage)
Apart from that I take omega 3s, and refined my diet, most of the calories from my breakfast are from protein, lunch is a balanced mix, dinner is protein

Generally more calories from protein compared to carbs is considered to be benificial

And the history as a whole
It's possible for someone who could study well during their entire school life, now has a lot of fun/procrastinates because they're "free" as an adult to explore
Whereas ADHD, generally the behaviour starts from childhood, they may not have reasons to "procrastinate" (or they may have a bad enviornment/family/potentially other issues), and they cant seemingly study till the last minute anyway
