I would like to apply to become a card designer, but I am not sure what my resume should look like. Should it look like a normal job resume, or does it only need relevant information? Like, does my high school diploma or previous work experience matter at all here? And for my portfolio, what counts as relevant stuff?

That's up to you. Your application should include anything that you feel makes you qualified for the position, that shows you're a good fit for it
