oh, medium/large scope is a vague description. Sorry. But in the project I used we had some integrations with external systems, I think 2 or 3 integrations. We had some complex business logic and something around 20 entities. And it would grow rapidly over time. I classify this as a medium scope. If you'll have very simple cruds, I'd go with nextjs only.
Ohhh that is a good example thank you! I was thinking maybe utilizing some kind of oauth2 google/discord for log in types and few entities in terms for the db stuff, do you think the complexity increases if i wanted to utilize amazons s3 also? cuz later i was thinking (maybe if i get enough time 😅 ) implementing a mobile app using flutter so separate backend would be good

Don't worry. There's no confusion. I think you're right on your decisions, especially if you'll have a mobile app. If you isolate your db stuff + s3 in a nest app, your next.js will be more clean. I think this is a good way
