A more in depth answer would be, that the game got dumped to PC, and has been played on emulator for a good while. Enough people have studied the internals of the game and understand how to spawn items and whatever else using the games own code. For a multiplayer mod you'd have to have some general sense of how to spawn another player model, and set certain statuses. Once you can do that all you theoretically have to do is program a network client that can send and receive data. Once you have both you can get data from someones game, and apply it to a spawned model in your own game

It really depends, but given that the game is just a bunch of machine code. If you study it enough, you can reverse engineer it. So if you originally wrote a function called Jump, then this function is converted into machine code when you compile. But if you know where in the games .exe this particular code exists, you can use any language to hook into the process of the game, and call that function yourself.

You can see this to the extreme in GTA or Mario 64, where they have literally reversed engineered the entire games code. To the point that you can download it, add your own functionality directly in the source and build it to a workable new version of the game.
