When we use Hold attack a cloud goes in that path we looking at. There are 2 things here The actual cloud and its shadow. Theoretically mobs under the shadow should getting attacked. But for some reason the shadow is useless. Mobs who get touched by the actual cloud are only getting attacked which messes with line of sight position we put the attack.
This a known bug. No official fix done yet

Observe how when mobs step in shadow nothing happens, but when the cloud goes through mobs they get hit
