With the current limit of three item slots, players are restricted in their ability to adapt to different enemy types and mechanics. Let's say, a Battle Chef uses his three item slots for meat cleaver, frying pan, and cooking pot. This is a standard build for the Battle Chef, but he will lack the damage to deal with melee-resistant enemies. With this, he can use the fourth slot with a shuriken, enabling him to deal with melee-resistant enemies better and expand his playstyle.
Another example is for the Sorcerer, who uses staff, book, and healing scroll. It is a good build, but very mana hungry. With this, he can use the fourth slot with a snare scroll to regain some mana and fry his foes more.

To preserve balance and progression, the fourth item slot could only be unlocked at level 100 and completing all campaign modes (Rookie League, Gladiator League, Champion League, and The Grindhouse). This fourth item slot would only be unlocked once you reached these criteria for a specific hero. For example, reaching level 100 on your Battle Chef would only unlock the fourth slot for that Battle Chef. Reaching level 100 on your Sorcerer would only unlock the fourth slot for that Sorcerer. Players who want to have a fourth slot on all heroes would need to fully invest in leveling and finishing the campaign for each hero individually.
We can expect Ultimate Rampage to be extremely challenging end-game content. Giving players increased build flexibility at the endgame phase feels appropriate. This ensures the additional slot does not affect early or midgame balance, while giving end-game players the tools needed to handle the increased difficulty.
