Long story short a bunch of Eldar prisoners supposedly started a riot that ended up killing several people. Once everything was back under control Vulkan saw that a woman who he knew and respected had been killed in the crossfire. Vulkan kind of snapped and so he lashed out at an Eldar child who was raising her arms to surrender and burned her to ashes.

I think it was later revealed that the Night Lords, who were also there, had intentionally triggered the riot and shot the woman to see how Vulkan would react.

And he also ended up regretting it, not that it means anything. Afterwards, when he's mocked by Curze for being just as violent and sadistic as him, Vulkan puts his helmet on because he's so ashamed that he can't even bring himself to look his own sons in the eye.
