We have examples of some Chapters literally just being created to guard a random stellar event to even bigger points of interest, so literally just come up with some excuse and you're good to go.

Successor chapters originally were decided by and actioned by primarchs. Now with most primarchs gone it’s decided by chapter masters of the original legion or the high lords. Some successor chapters like the Black Templar successors can be made by crusades/companies who get split off from their chapter and need to form a new successor.

Id also HIGHLY suggest reading the fandom wikipage of the Chapters in the Badab War.

ıts great because they just copy pasted all the texts from the books and it extensively covers their political baggages.

Imperial Armor was very good at establishing the things it takes to run a Chapter and gets into the guts of the deal

compared to modern lore where it makes it seem like marine Chapters can largely do what they want

But the successors of the Black Templars were a very rare case of how new successors are made as usually the successors are of the original legion and not another successor of the legion.
