so you'll probably find some high ranking guard who's like 80% augment from battle damage

The Imperium is pretty aptly described as akin to the Holy Roman Empire in that it's more like a patchwork of a myriad of vastly different societies that are bound only by this loose idea of the High Gothic/High Imperial culture
That seems to only exist in the void between worlds and the few worlds that are directly connected to the central bureaucracy (like shrine worlds)
And obviously Terra itself
Unless otherwise stated though, it can be pretty reasonably assumed that a world is dramatically different than others in the Imperium

The other (much more cynical, but arguably even more accurate) description is that the Imperium is a hyper-colonial apparatus; the "High Gothic" Culture is essentially Terra exporting its values, but like any Colonial Empire, the colonies themselves are always going to be quantifiably different to both the Imperial Capital and other colonies
The Indian and African colonies were nothing like one another, and were in turn nothing like Britain
The reason this model is arguably more accurate is because the Imperial Tithe, the essential universal constant in the Imperium, is stated to be quite literally "solely for the benefit of Terra"

Thoughts? I want these items to be strong but to have penalties/drawbacks or be potentially dangerous with each use, seeing how it’s xenos tech and it’s necron xenos tech being used by humans

Even the lowest ranking arbites has regular access to space marine vehicles and sos level equipment. Crusader houses are really rare and extra-elite protectors of lord inquisitors and cardinals.

Yeah its why I like the custodes change, they are gene-crafted to be the top form a human can achieve and are like greek myth heroes in some sense, so the unisex approach makes a ton of sense compared to Space Marines who are just a blunt hammer tool.
