The Titus situation really highlight how SM fans see themselves from the wider Imperium.
Leandros had quite a few reasons to think that Titus was strange and might have some Chaos corruption.
And if the Codex does say "In case of Chaos suspicion, don't warn the Inquisition but deal it internally with the Chaplain", then the Inquisition, aided by the Custodes, and most Imperial Guard regiments would start to decimate the SM. It's just insubordination at the very least, or heresy at the regular

The book was written before the Inquisition were a thing..
So while going to the Inquisition is definitely something to do, it's not the first thing.
First step when you suspect it, is to report it to a Chaplain.
The Chaplain is then the person that decides how to deal with it. Require penance in some way (usually things like going into battle without ammo or with striped down armour or something),
Can determine there's nothing wrong
Could execute them themselves
Or report to Inquisition for further investigation.
The Marines do a lot of things the Imperium would consider Heresy, but they're marines, what you gonna do about it.
For one, most of then hold to the Imperial truth and don't believe emps is a god
