The reality was it was mostly a last minute editorial change from the higherups but it was his name on the forefront and the very first page of what was essentially incidental wank material and since this is 2010 40k fandom they ridiculed this guy to absolutely ridiculous heights down to even giving him threats/harrassment mail.

Ever since then GW has entirely stopped crediting people in rulebooks.

It was not his fault essentially but people just didnt care, Ward literally helped write shit like Vermintide 1/2 and even DT btw lol

Almost any of the “human looking” characters in the GC list could be chosen to survive, though I would think it is most likely below Magus/Primus in rank (since those are needs to be the public face of the cult).

If going with the “legendary insurgent” type, most likely the hybrid with 3 pistols. The lore for that kind of runs on that mythos archetype.
