(1) Yes, and no, simultaneously. Machine spirits do exist in the higher end complexity war machines, but for the average civilian household item, the “machine spirit” is just a thing people beg to spiritually for hope they can get said appliance to function properly.
(2) Titans have a pilot that, unlike a dreadnought, can actually leave the piloting seat. The pilot does have a lot of augmetics to mind link with the Machine Spirit of the Titan.

To give an actual answer; Machine Spirits are actually a thing but what they are is left up in the air for writers to play around and have fun with. They are either:
- Actual machine spirits
- Leftover AI
- Or finally the influence of the C'tan of the Void Dragon lumbering under Mars.
Titans operate by a Princeps, the amount required changes from Titan to Titan but you basically need to have immense willpower to work with the machine-spirit of the titan and cooperate with it before fully managing to control it fully.
