Hive Cities go to the stratosphere; they're the size of a continent and hold the population equal to a world. We can see Tertium from the MourningStar. It's right there. So if there was a second one we'd also be able to see it (given the mourningstar is not in geostationary orbit, but even if it was it wouldn't be hard to just go to the other side of the world) if approaching the planet from the other side. Unless the 2nd city is in ruins, or God help us underground, there's no way it'd be a secret (given, most people in the imperium never leave their planets, but we're off-worlders, save for the Adeptus Arbites and hive scummers).

Like it would make sense that Grendyl or Rannick is forcing the Mourningstar in geostationary orbit so we only see the current hive city, and can't see the other.
(||whereas what we see is a skybox made due to development resources being limited and more or less a static image with moving assets placed over it||)
