havoc is a challenge mode that adds increasingly difficult modifiers as you progress through levels from 1 to 40. Level 1 through 10 are based on malice difficulty, then heresy for 11-20, then damnation for 21+

You can only do them if you're level 30 and have completed a qualifier mission in the form of a maelstrom or auric maelstrom mission which give you access to havoc 1 or 16 respectively

and people are probably denying you because you're trying to apply to ones that are too high level, or because your account is very new and its a challenge mode and they are using modes to eye ball things like that

or they just don't want that class

or someone else applied a second before you

If you're 30 and want to do the qualifier, you talk to dukane and she'll recommend you do one of the two to get started. Once you have qualified, you'll have your own havoc mission at either 1 or 16. Every sunday you'll get a reward for whatever the highest havoc mission you've cleared is in the form of dockets, plasteel, and diamantine. you get a little exp for each havoc mission completion but this big lump is doled out once each sunday.

As difficulty increases, you get more and more modifiers and they're more punishing, like giving you less hp and giving enemies more hp and damage beyond the normal difficulty modifiers for malice, heresy, and damnation

If you run scum however, as rampage is in the mix you are basically guaranteed the rampage blessing, and vet toughness reduction which is really, really strong
