Heavy spam for hordes, charge the heavy if you have time since you use ironhelm but yes TWBS isabsolutely necessary if you dont want cock and ball torture

Avoid the third heavy on ironhelm unless it's hitting an elite/specialist, so basically after the second heavy you do a light and just alternate light/heavy

All 3 options are good but I favour incendiary over stuns. The throwing knives are a specialist tool, you should know if you need them (either if you lack a ranged precision weapon or if you have a Flamer or a Bolter with the super slow draw speed, because you can skip the draw with it
