Update to the newest watchOS 11.4.1, that is a very old and early version of watchOS 11.

Also make sure iOS on your iPhone is updated to 18.4.1 as well

Every once and a while my iPhone and HomePod Mini alarms just don't work

It's super annoying

Don't use the Apple Watch alarm much though, mostly just timers

The alarm is built into watchOS, and it's possible that early watchOS versions had some bugs with the alarm. Updating watchOS may fix that issue and more.

I would highly recommend updating your devices.
