Help with setting up Dynmap to MySQL | Dynmap® | Page 1

I'm renting a MineCraft server at ShockByte and have created a MySQL server.
I'm trying to get Dynmap to connect to MySQL but it seems like I might be missing something.

I've managed to get another plugin to connect to it so I know it works but I am not sure I am doing Dynmap setup correctly.
I also noticed that when I change the "type: filetree" to "mysql" it reverts to "filetree" after I have started the minecraft server.
