we don't use beta blockers in acute chf
in chronic chf, we give b-blockers
we don't give digoxin in chronic chf coz it increases myocardial contractility, which we don't want in chronic chf conditions

in acute chf- we aim for reducing edema, increasing hr and increasing myocardial contractility
in chronic chf- we aim for reducing the load on heart, by vessel dilation and reducing cardiac remodelling (common in long term crisis of chronic chf)
metoprolol is a beta blocker---> used in chronic chf ---> coz it reduces heart work load
digoxin althought has vagomimetic effect (reduces hr), it also increases myocardial contractility which is indicated in acute cases but not in chronic chf

Hi a-a 👋 so as you know metoprolol (b- blocker) can not be used in decompensated HF… first when someone has lets say, functional cardiac disorder or any cause that leads to reduce ventricular filling or ejection of blood——-> then decreases cardiac output ——> Heart failure. that decreases in CO also causes an initial drop in blood pressure——> triggers a compensatory mechanism, like the body wants to save the other organs bec there is not enough blood coming from the heart. The compensatory mech. is by activation of sympathetic n.s and RAAS, and that would be so helpful for the other organs and organs will get enough perfusion but the problem is that this compensatory activity that will save the other organs over time will hurt the heart ( and the heart is already has the main issue) …sympathetic, RAAS and ADH if it stays stimulated for long time, will lead to chronic stress to the heart and something called ( deleterious remodeling)——-> then decompensated hf occurs ( the heart gets so tired from that activity and even will no more responding to that compensatory sympathetic and etc.) and can’t even work properly so now we are in a state that we wish that this heart can respond to sympathetic stimulation so it can increases its heart rate and contractility not the opposite by giving him something to block it

So that’s why when the heart failure is still ( compensated) , part of the management is to give the patient beta blocker, to block that sympathetic activity from the beginning ( to reduce heart rate and myocardial oxygen demand and also decrease the vasoconstrictors ) that will help to focus on the tired heart already and prevent that state that called ( deleterious cardiac remodeling= decompensated hf) and thats why we call beta blocker ( cardioprotective) . But once the patient stabilizes from the decompensated state then we slowly start to give them the beta blocker again 😁

A good way to memorize it is angiotensin II is part of your renin angiotensin aldosterone system which wants to INCREASE blood pressure

it does so by vasoconstriction, stimulating aldosterone (which increases sodium and water reabsorption), increasing epithelial sodium channels directly (increasing water reabsorption) and lastly increasing ADH because there are are AT1 receptors in the brain.

hey nas 👋 no, angiotensin II will not decrease blood osmolarity... so if somone has vomiting or hemorrhage( or any cause of losing water and electrolytes) or call it volume, then the body will do everything to retain that volume(water and electrolytes) so yes angiotensin II will be activated and will stimulate ADH too ( to not letting water go out thru urine, we need that water) but at the same time angiotensin II will activate aldosterone to reabsorb the sodium and not letting it go out too bec the body already in need of that water and Na TOO (to increase osmolarity of blood)...BUT if someone is in the desert and has no access to water (dehydrated) = his blood is hyperosmolar= concentrated with the Na bec no water to dilute it, do you think the body will activate angiotensin II? No, that will lead to adding more Na to blood and the body don't need it, but in case of dehydration or hyperosmolarity of blood, the brain has receptors telling him that we need more water, so these receptors will send signals to pituitary gland in brain to stimulate ADH hormone to stop the kidney from excreting that precious water in the urine for now, and reabsorb it. but only ADH will be stimulated not the RAAS system, we don't need Na in case of hyperosmolarity states

Diuretics are great drugs they can massively reduce oedema and nitrates are always a drug to be given in case of problems related to heart , even some nitrates, others such as nimodipine are still used as best drugs for heart related issues

Whenever any q related to heart comes i always envision all the most common drugs used in it and then i segregate what drugs are given for this particular problem of heart , it becomes much easier

hiii 🫂 so I'm not sure what is exactly your level and tbh I studied EKG many years ago ☹️ and now we don't use calculation, like we dont actually calculate the number of squares to see if there is short or prolonged PR and els....but from ur q, I think you're missing the basic, so I'll try to explain that according to what I learned from dr. Rayan(bnb) long time ago, also I will share a file that might help u hopfully

we have 12 leads EKG, and it is 12 bec there is 12 different recording of electrical activity in the heart, the leads each have a name

look at that pic, lead I , II, III always showing in the left side

then comes leads avr, avl, avf..... followed by v leads = v1, v2, v3, v4, v5, v6

at the bottom of the EKG there is often one or more leads we call it ( rythm strips).. it usually lead II and in that pic, u can see lead II and v1 at the bottom( and these are multiple QRS recorded in a row to make it easier and better for the the physician to read the heart rhythm( itis like a brief)

also there is expanded lead II at the top so we can see the p wave and other things more clearly

When they put the electrodes on the chest and connected it to wires then to the recording machine, u know that each electrode placed on different places, bec the electrical activity in the heart moving in lots of different directions, but what we want is the sum of that electrical activity in the left ventricle

#

if u look at that heat, u can see the position of each lead, also look at the line with he arrow ( \ ) it showing that each lead observe the electrical activity from defferent view.. and thats why normally lead II show QRS pointing upward, and QRS of avr lead pointing downward

#

bc avr lead looking at the electrical activity going away from it so qrs will be pointed downward.. now first thing i look for quickly when i request EKG is the QRS of avr lead, if it is upward, then that means that they put the leads incorrectly and they should repeat it.

u can just imagine those leads like persons looking at a car(electrical activity of the heart) , so person avr will see the car moving away from him, but person lead I looking at the car coming towrds him

so if u know the normal EKG, then u will definitely pick any abnormality

like for example: you know that the P wave represent the electrical activity that comes from the original signal or impulse from SA node and associated with atrial contraction, so it will make sense to u if any abnormality of the p wave like absent p wave or if it altered, that means the cardiac impulse originates from outside the SA node (abnormal) .. That's an easy example of how u should know the anatomical locations

Hyperalgesia
Is a sign or feature. And it is one of the signs of CRPS. You say that the patient has hyperalgesia if you do pinprick and the patient exaggerates the feeling of the pain. Normally we feel the pain of pinprick and we might just say OOUCH 😁 but if the patient screams loudly then it’s HYPER-algesia. But still we need to know why he has that sign right? So it is not a disease in itself

Complex regional pain syndrome,
from it's name is a syndrome, which means : many signs and symptoms that occure together.
I'll give you a case and I think it's the best way to keep it in our mind
a man was moving his furniture in his office then he hit something and he felt a stab in his right knee, but there was no immediate deformity or swelling or any severe pain and he was walking normally. then after 2 weeks he started to feel severe pain on his Rt.knee, and could hardly walk, he said that even placing blanket or any clothing on his knee will make severe pain and discomfort, so that what brings him to the clinic, then they gave him strong pain med......
but over 2 months he starts to have swelling, increase temperature, and sweating around it, and skin spots but the left knee was normal.. they did the pinprick test= positive, which is one of the signs. but there are more signs..
The reason of this syndrome they think is that due to injury--->inflammatory chemicals and pain peptides(protiens) released by peripheral nerves----->increase sensetivity to sympathetic nerves---> then abnormal response and sensation to pain.
so they call it complex cuz there is a gap of time between the injury then the appearance of signs and autonomic symptoms.

yes it is point of contact of blood vessels afferent art(has juxtaglomerular cells) + DCT(has macula densa cells) and of course the main cells mesengial cells...

and yes u are right it helps to regulated bl. pr. and GFR.
like in case of hypotention= there will be decrease delivery of Nacl to macula densa cells and that will stimulate RAAS ,

Depends on how much time you got , if it's very short time and a lot of chapters , sacrifice some of them, ask your classmates what are the important chapters you should focus on, and do spaced repetition, for example try to memorize one paragraph , then move to another one , after finishing it , try to recall the first paragraph by writing it , and the information you didn't recall, rewrite them with another color and yk those should be the one you focus on , this is what helps me when I have very short time left for my exams

Hello, I am a second year medical student and I am struggling a bit with lectures. I find myself taking hours to finish studying a lecture only to find out next day I cannot remember most stuff the next day. How can I avoid that? I mainly study by annotating and asking myself while reading the slides. I tried anki, but making cards take a long time and when reviewing, my card speed is about 3 cards per minute. making flash cards is actually a method of studying on it's own, it's basically the same thing as making notes 🙂 but spaced repetition is the whole point of flash cards, reviewing it at longer intervals over the course of a week or month to force your brain to realize "i'm seeing this often, i should remember it" - eventually, the topics you've seen multiple times will come back faster, you'll be doing cards at a faster rate, and you can make the judgement to rate the terms you know as "easy" so that you see them less which gives you more room to do newer cards

if it takes too long to do, ask your classmates if they made any flash cards, look on google, or use AI the way it's intended to be used: make a list of terms and definitions, and ask chatgpt to turn them into an anki deck

But i think the best is that you go ask some student from the schools you can go and contact some real dentist/pharm! In France for instance we have a good "alumni" system where you can contact student easily, and for the pro, at least in pharma, i'm sure they'll be very happy to tell you if you go to the drugstore in a no rush time ehhe

i studied all my life by writing lessons at school then reading it again at home .... now when i went to the medschool i found my self rewriting all my pdf lecturers then reading it from my notebook because it better for me to memorise the things with my hand writing and i also started passing the exams and not failing at it .......... is this how my brain work or am just not used to studying using by printer or pc screen

am asking this because writing takes sooooo much time

Yes our teachers always gives us workbook whee only 10% info is added like images so we dont waste time making diagrams and have spatial memory, try using ipad with your pdfs, or some other tablet drag the images to your notes area and you will have much better spatial and recall memory.
Use ai to make questions for yourself on your competitive exam pattern . It helped me massively in med school especially during scheduled test in our internship. Trust me brother start doing it because lots of things are inage based and you need to see the image with the notes .
Even laptop is also fine. A small mini printer is best for physical notes if you dont like tech, i bought one in china and it worked wonderfully and is dirt cheap and is smaller than my hands, so its portable for you in library also.

If you don’t like that also try using textbooks, let say u study congenital hypertrophic pyloric stenosis, you need to see xray photo, you cant make one, go to your book see where the page is and mark it and in your notes (page no) this is tidious process.
I will still prefer the mini printer as stickers also come with it you can stick the diagrams on your notes . No ink is required and u can put a tape on your stickers so it doesn’t vanish with time

Hey everyone, I'm not a med student but I'm writing something for school about neural plasticity. I had a question about gray and white matter. Basically so far my understanding is that the majority of gray matter is neurons, and the white matter is the synapses aka the connections with the myelin layer and stuff. I don't quite understand: is the white matter 100% synapses and gray matter 100% neurons or do both have neurons AND synapses in them? Thanks in advance for the help (image is of a brain)

Regarding to neurons, so first, one neuron has 3 parts
1- cell body 2- dendrite 3-axon...
it is like head with hair (cellbody and dendrites) and tail (axon)..
The cell body with the denderites of the neuron is located in the gray matter of the brain, not the whole neuron. And the axon part located in the white matter of the brain.
The synapses mostly happen in the gray matter because, denderites synapses with another denderites of other neuron there.. then the information will travel from denderites to cell body of neuron to the axon( in white matter) then to its destination, so axons in white matter is just travelling or taking the info rapidly from and to the brain, rarely synapses occure in white matter... Also axons can be so long to reach to the foot for example, like it's 1-1.5 meter.

so I drew this bug and a foot about to step on it to show how the brain makes the body move by only 2 neurons, one neuron from the eye and other from the brain..First the eye (sensor) sees the bug then sends info to the brain. In the brain’s gray matter, you have the neuron cell bodies with their dendrites (like little hairs). That’s where synapses happen then denderite will take the info to cell body.
Once the decision to step on the bug is made, the signal travels down the long axon (the white matter) then into the peripheral nerves and finally to the muscle fibers of the foot. That’s why the foot moves.

I have to qualify my home country one first, i am in my country rn, china suffering is over thank god . I dont even want to study there anymore and nor recommend anyone to study there. I went there to study medicine but it felt like i was studying language course which wont be helpful to me ever in my whole life. We were promised for english course, we got it, but then they forced us to pass such a difficult language test (even internship was forced) that would make us speak and write fluent chinese that too during mid internship, suddenly in one day bypassing their own law. I sweat so much to pass that test, but failed my licensing exam with 3 marks because of no time to study. Also 15-20 peeps in my university cant get their degree even after passing home country test why? Because they cant pass language test of china. Its harder than anatomy. The amount of rules they change overnight is insane . Luckily i escaped 😭😭 and got my degree also @proud sable maam🥹🥹🥹

The work culture and ethics are also the best. Even certain hospital provide free 250 dollar card every month which can be used to eat food, free macbook and research material 😭😭🥹🥹🥹 so i dk why people complain

Yes , atleast in usa doctors have to follow strict ethics They wont scold u in front of everyone and in my country as well as china, you get scolded hard and treat like slaves until your residency. In china one resident came 5 sec late , she was openly scolded by the director so badly in front of everyone. She was walking behind the group of her colleagues and she was about to cry and then she has to work 70-90 hours a week, get scolded to get paid like peanuts. Oh yes also the patients are over emotional here so they can beat you , so u gotta be master of conversation here .

Its fairly more strict here i mean people here think they are better than u especially in asia where they think they are the best so your seniors will bully you , till this date i have 2 seniors close to me rest nobody deserves my attention for being a tryhard bully 。Mess with the professor and gg he/she will never pass you so you have to face bullying from them and become their slave, but they also have insane amount of knowledge. Its same in china also where i saw two doctors fighting openly. But there is one thing that chinese and indian docs have lots of clinical skill due to heavy load of patients . They have very vast knowledge . The knowledge bullying thing is so much here that you also become a knowledge bully like your seniors and bully your juniors

Everyone is jealous of you if you perform really good i dk why people cant focus on their own lives. The pay is good i dont say its low its actually good , the medical entrance exam is very hard here to enter med school as well as residency but i feel usmle is the hardest because in asia cramming heavy amount of material matters whereas in usmle concepts matter. Also in 90% asian countries doctors dont know how to speak english . There were like 3% chinese doctor who actually spoke good english , 2% belonged from private hospital , so as a foreign student its hard to study under such doctors

Oh one more thing @reef yacht the medical scam, you cant do anything about it. So my friend had small navicular fracture in china, first we were asked to pay lots of money for appointment which is bs , doctor asked him to pay 10000 yuan aka 1404 us dollars and wanted my friend to stay in hospital for 10 days, just for navicular bone fracture? If it was not a medical student he/she would get scammed. Luckily my friend went to his professor and the professor recognized us and he just added small cast worth 300 yuan aka 41 usd and my friend recovered in 3 months easily. Now same with another lady whose baby was declared dead and money was extracted ,later she found out her baby was not dead. I broke my hand one time, our uni forced us to buy insuarance every semester but i was never paid anything nor my friends who had bigger injury, even my friend here who had navicular bone fracture no refunds. Oh also patients are too emotional here so they beat doctors mostly surgeons , saw a patient grabbing collar of senior surgeon in china, Its the same in korea , japan mainly , russia and many other asian countries. This is problem with healthcare here. 🥲 thats why many docs you will see in america are immigrants mostly from asian countries because their skill is much respected in america .

I have to write a paper where a certain philosophical stance/theory is being linked/compared to a respective scientific point of view regarding the topic of the paper.
I have chosen for time, and more precisely: time-consciousness.
As of philosophical frame-work I intend to use Husserls' account of time-consciousness.
As of the scientific frame-work I was thinking about making use of the current debat in neurological frameworks regarding how people perceive time, but also objects.

I finally managed to save up about $16k to start an LPN program — classes are Monday through Thursday from 4–10 pm, and 2 of those days include lab. For me, mainly just practising interpreting EKG based on theoretical study. So, after studying I used to do practice ECG's while I always had a normal reference EKG besides me. This really taught me to find the suble differences and to differentiatie between normal - abnormal even if subtle.

Sometimes, just googling helped. Often, I used en.ecgpedia.org (originally in Dutch, but it's also translated) and videos.
Important for EKG's is to have a steady sequence in your interpretation, e.g. rythm and regularity -> bpm -> heart axis -> intervals (PR, ST, QRS, QT, R) -> segments -> wave progressions ....

We got the practice ECG's from the professor, not sure what sources could provide practise questions though... Someone else would maybe know that.

if you sleep 7 to 8 hours then that is pretty natural, and you should never try to sleep less because it will hurt your focus. however if you sleep more like 10 hours or so then that maybe a 1 time thing or it maybe a more serious issue

a general rule of tumb that i always use is to ALWAYS wake up at the same time. even better sleep at the same time. because that sets your circadian rythem, and try to get sun light in the morning as soon as you wake up (get sun on your eyes but dont stare lol) because that basically signals to your brain that it is wake up time so your monkey brain stops releasing melatonin and the cortisol levels rise. and it helps locking in your circadian rhythem. it also raises the dopamine baseline for the day

this works best within the first 30-60 minutess after waking up, and if there is no sun outside its still really benifetial, even on a cloudy day. because it signals to your brain that its wake up time

hello! psychology major here. in psychological terms, the term variable means anything that can change and that change can be observed. theres usually 2 kinds of variables used in the experimental method but thats a different topic altogether.
the sentence you shared talks about what makes the experimental method in psychology different from other methods, which is the fact that the experimenter can control the variable.
in terms of the sentence structure, the subject in the sentence is 'the ability to exercise precise control over a variable', verb is 'distinguishes' and object is 'the experimental method'. sometimes in sentences there isnt just one subject word but rather a phrase (group of words) that become the subject. i hope that makes sense!

salamu alaikum, idk if you got an answer but failure in medical school doesnt mean failure in life. if you still want to study medicine and become a doctor, (im sure you know this) you can still try in another country if you can finance it.
i fought so hard to get into med school .i thought this was my purpose in life and i would rather die than not get into med school. while it is something special, i learned with time that it doesnt define me and certainly is not something i would want to do all the time because i have so many other interests. so if i would fail, i would cry and probably have to deal with disappointed family but in the end i would start something else. i think the mental pressure is the worst part of it because we think if we dont live according to the standard of hussle mentality, we are not good enough and give ourselves the "failure" stamp. but i mean, as long as you live, you have the chance to do anything really. it needs time and patience and money and definitely confidence.
