Create a todo list w/ a realistic amount of the tasks that u want to get done , write down how u will get the tasks done (break the "study math" task into lets say, solve 3 exercises from chapter 8, study theory till I can say it out loud with ease) , then quickly create a plan for the day that will cover when you will do the habit of doing that itself and putting X (not done) in each task in the end of the day will make y realise slowly and nicely why procrastinating isnt something you wanna be doing. And I'd say, apart from a habit like this which has to do w/ practical stuff that y get done w/ ur hands , get into good thinking patterns as well. Like, ask why you do what you do each day, ask urself what you want to achieve, ask yourself what will happen if you dont achieve what you want and in general, reflect a lot through out each day.

try to gamify study, adjust ur environment, mess around w/ apps like forest

and get an accountability partner, preferably some1 from ur school who cares about his future

but like most important part that i forgot is to probably, schedule how you will prioritise spending ur energy first and then your time

and u prob waste lots energy without even realising it on social media, too much music etc
