They do an additional interview if they believe there was variability in your interview performance and an additional strong interview would make the difference between hire/no hire.

I believe I had to do the additional bc in one of the technical interviews we got sidetracked and didn't get to the last follow-up question. The additional went well, I was able to get the optimal solution to the last follow-up

It depends, in that particular interview there were three different questions total that built on each other
