Tackles are currently cheaper than ATMs (3.3 versus 3.8 rates). You'd be able to get more quantities of Tackles over ATMs.
Both of these providers actually gives about the same profit from a full farm, but you'd have to harvest ATMs more often (2 ATM harvests is equivalent to 1 tackle harvest)

It depends on the condition. If there are any reds (considered as high tier and requires immediate attention) conditions, you should solve them first before doing anything else.

Let's say there's a T3 bleed (red bleed, patient is losing blood very quickly!)
You should bandage up the wounds.
There are several ways to do this:
Anesthetic > Scalpel > Stitch (will stop all bleeding immediately; does not account for sponge prompts)
Anesthetic > Scalpel > Clamps (Transfustion whereever necessary; does not account for sponge prompts)
Stitch to stop all bleeding (does not account for sponge prompts)

After stopping the T3 bleed, you should be able to complete it like a normal surgery.

Another example, lets say the patient has their temperature at 107.3 (red, requires immediate attention).

Use a Lab Kit in order to unlock the Antibiotics tool, then spam Antibiotics to reduce the patient's temperature.

If there are no reds or anything which requires immediate attention, then use an Ultrasound to diagnose the patient.

Both of these are cheap as well and the total of these three should still cost lower than that of the price of a Mechanical Butler.
