I find the game becomes very unstable when doing quests, especially if you swap equipments across characters. The game would freeze when restart a quest, then, randomly detached their equipments especially cairo guidance, where i only put on one character and i never swap.

It's actually a problem with presets I guess. From what i have observed, presets not only save the equipment on your team, but also some from characters off the team as well. So when u load a preset, ur quest character will get the equipment it had when u saved preset, although that character was not on preset team. That's what is happening with me i guess
