So here is the answer, I tried several times and I'm sure about that now:
PD effects on gear do stack, 2x 50% chance for 33% PD results in 2 checks for success, they can stack and cause the double effect, while life conversion works differently. Once you gained the effect, it cant be triggered again.

Still not sure about dmg reflection but i guess the effect cant be triggered twice at pne time, so remains one question, does the chance raise when using multiple reflection items?
