Yeah, seems good. Though, not sure why you included several forms. I usually write them like this.
Green (D is a region in ℝ^2):
∫(Pdx + Qdy, ∂D) = ∬((∂Q/∂x - ∂P/∂y)dxdy, D)
Gauss-Ostrogradsky (Ω is a region in ℝ^3):
∬(F·dS, ∂Ω) = ∭(div(F)dV, Ω)
Stokes (Σ is a surface in ℝ^3):
∫(F·dr, ∂Σ) = ∬(rot(F)·dS, Σ)
