I have personally NEVER activated last meal because I never see a need to do it, but just out of curiosity i decided to activate it for fun just to see what would happen.
It took a dragon fruit I had in my inventory, and in exchange I got half a heart.
I'm sure you can already guess what I'm about to ask for.

AFAIK all it does currently is just check when you get hit on your last heart, if you have food it'll pretty much just ignore that damage and delete the food
And I feel it makes sense not to heal you, since if you ever get that low you should just eat it normally instead of on death

the fact it deletes the food to let you completely negate fatal damage makes it incredibly powerful
