Third difficulty adds a buff to enemies.
Fourth difficulty adds another.

This time around we have Defense Drill. Meaning that you have to defeat a huge swarm of weak enemies. Their Defense type is Elastic so you need Purple AOE DPS units here.

Third difficulty buff reduces your ATK each time an enemy dies, but removes some of the debuff whenever you are healed. So you ran healing that ticks continuouly, rather than big one time heals. So like Onsen Nodoka, Hanako, Hanae etc.
Fourth difficulty this time increases enemy HP even more.

After you are done with 3 teams the game sums up your scores from them and gives you currency according to your score.
You can buy Fusion Keystones (good stuff), Tsurugi, Eimi and Sumire Elephs. And Secret Notes.

There very first time I did the reset, I think I spent like 250-400 Pyrox total. Got rank 1, came out with like 40 pulls I think?

It's just something to consider.
If you just do 1-2 refreshes, you are wasting Pyrox. It's only worth it if you are shoot for a score higher than normally possible for you.

Basically how it works is, when the brackets are reshuffled, the game doesn't automatically place people in brackets. Only when you first log in. That means before that the whole bracket is just level 20-50 bots with shit lineups you can't lose against. And everyone starts at the bottom.
So some people keep refreshing for seasonal first time placement Pyrox from positions that they can't normally reach.

So you do this very exciting gameplay, where you sit in front of the login screen, waiting to get in, instantly go to PvP, then refresh opponents for big placements jumps. Then you buy tickets and repeat until you are satisfied.

Atsuko is a goddess is you are running 3rd or 4th difficulty, because her healing is up for ages so she can keep Hanako's ATK debuff from highest difficulty low or near 0.
