If someone's going insane you want to exaggerate their eyes to be really tiny, this is a super commen technique used in cartoons to show like distress or a feeling of anxiety, like the smallness, the contrast is signaling something big is about to happen, so just absolutely shrinking the iris will help. Another trope about insane people is they like, laugh for no reason, so making her smile without her eyes, can help even more, because you can tell its not joy shes feeling, but she cant help but smile, which feels unsettling. And lastly, just adding some basic shading, giving a dramatic feeling can enhance just the line work, and with a black and white picture you drew, adding a pop of super saturated and bright colour will push the contrast even further, and adding that same colour to the eyes will just like bring it all together. It also gives murderer vibes, so idk if thats what you want, but I went off what you said you were going for

i'm not adding color like that just yet cuz it's a character design i'm making and idk how to explain but i want her to look like she's like- going insane but also numb if that makes sense ? like she's so down the rabbithole of insanity that she feels numb so that's why i went for the huge eyes instead, i didn't like the small pupils on her at all even if it shows insanity really well
