Make sure you're not appending the bucket name to your custom domain.
ex. if your object name is cookies/pineapple.png, you would visit it via your r2 bucket custom domain https://r2-bucket.example.com/cookies/pineapple.png

That looks like dns cache? Did you have the domain pointed at something else/recently changed it to point at r2?

Looks like most DNS Resolvers updated besides a few ISP Resolvers
You could try clearing your DNS Cache (Depends on OS) and switching your DNS Resolver to 1.1.1.1: https://1.1.1.1/dns/
Those are local changes though, eventually dns cache will expire if you wait long enough, might be a bit though (few hours, prob, some ISPs ignore record the ttl of the record/max time it should be cached, and cache for longer)
