most likely a wiring issue. All your wires are completely unshielded and that board is very susceptible to data issues. If you look at some of the other ADXL posts I detailed how I wired mine up using a ethernet cable and ensured the ground is twisted with the data lines which makes it much better at transmitting over longer distances
Less likely, your board is DOA

As just mentioned, noise can be an issue. As well as a network cable (twisted pairs) a 3.0 USB cable works great. has multiple pairs of twisted shielded conductors and is very flexible to boot. either or work just fine.
Just tried my old short cable, suddenly it works.
which points to a data line issue. The problem becomes more likely as the cable length grows unless you shield it somehow
