you're probably going to have to take the ielts anyways unless you're from an english speaking country so take esl
I could probably get a 7 in EFL
But defo a 9 in ESL

Hello, I have done some researches before because I was in a similar situation as you are right now. If you are a citizen from an english speaking country and own the passport, you are fine whether you take esl or efl. If not, you will be doing the ielts anyway even if you took efl unless you go to university before 7-9 months after you have written your efl igcse exam.
Take into account that different medical colleges requirement's vary
The only thing they care about is if you passed it or jot
Along with oral endorsement
For medicine you need chem bio and math
